Care Manager: Northern Michigan Regional Entity (NMRE), a Prepaid Inpatient Health Plan (PIHP), is seeking a Care Manager to answer inbound Access Center calls, provide screenings and make referrals for appropriate care. Experience in substance use disorder treatment or mental health care is required. Bachelors’ degree in human services or a related field from an accredited college or university with corresponding state of MI licensure and/or CAADC/CADC is required. Masters’ preferred. Position Overview

Assist individuals seeking services in a fast-paced access to care call center environment. Ensure consumers seeking services have access to and receive necessary, clinically appropriate care in a fiscally responsible and cost-effective manner. Identify needs, determine levels of care, and refer individuals to appropriate providers and community resources.  Provide care coordination, referrals, follow-up and exceptional customer service to callers.  Maximize the supports available to enhance treatment retention, engagement, and recovery outcomes.

Qualifications

  • Bachelor’s degree in human services or a related field from an accredited college or university required. Masters’ preferred.
  • Corresponding State of Michigan licensure and/or CADC/CAADC is required.
  • Experience in substance use disorder treatment or mental health care is required.
  • Lived experiences with behavioral health are valued.

Essential Job Functions

  1. Answer incoming calls in order to gather appropriate information and provide referrals for individuals to all levels of care to substance use services, community partners, health homes, access departments, and customer services.
  2. Provide access phone line coverage in a timely and efficient manner to ensure exceptional customer service.
  3. Provide a welcoming, accepting, and helpful experience to all individuals requesting services and demonstrate empathy in crisis situations.
  4. Utilize the ASAM Patient Placement Criteria for level of care determination as well as authorizations and utilization reviews for substance use disorder treatment per Prepaid Inpatient Health Plan (PIHP) contract requirements.
  5. Adhere to Michigan Department of Health and Human Services (MDHHS) Access Standards, Medicaid Provider Manual requirements, PIHP standards, and other applicable regulations.
  6. Refer individuals to the most appropriate provider for assessment and treatment and provide follow-up care coordination.
  7. Provide care coordination addressing all aspects of consumer’s care, integrating behavioral health, primary, and specialty medical services, as well as other community services.
  8. Maintain customer relations with consumers, referral sources, and stakeholders.
  9. Utilize motivational interviewing skills.
  10. Serve as a contact person for consumers in need of advocacy and support in securing services that support recovery.
  11. Manage access center related support tickets and assist providers when needed.
  12. Obtain and review after-hours crisis reports.
  13. Perform other duties as assigned.
